Contact Form 7 forwards a record of your form submissions to our Practice Manager via email. Your data may be deleted by the Practice Manager. You may request a report of saved data related to your email address.

Cookies
Forms related cookies : When you submit data to through a form such as those found on contact pages or comment forms cookies may be set to remember your user details for future correspondence.

Who we share your data with
We do not share any of your distinct private data such as name, email, phone number and medical conditions, that you mentioned in the contact form.

How long do we retain your information
The data that you enter in the contact form are retained and stored in a secure server indefinitely unless you ask us to delete them.

What rights you have over your data 
You can request to receive the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

We store the data that we collect from the contact form in a secured server which is located in the United Kingdom.
